The Leaf Crest Medallion is an advanced from of currency for trade with the Satyrs. They can be acquired through trading high-end drops - usually attained from fighting elites or named bosses at Hel's Temple, beyond the Arcadia Camp - not with the merchant in the cap itself. Trades are usually limited to one trade per item but sometimes they will do multiple trades for each item type.
